Thank you for your interest in becoming a Temple of the Goddess Affiliate.  Please allow six to eight weeks for processing once your application is received.  There is a non-refundable application fee of $100.00.  You may either apply by mail, or online using our secured PayPal account.

To Apply by Mail, please:

1. Print and complete the TOG Affiliate Application.

2. Mail your signed application along with a $100 check payable to Temple of the Goddess to:

By submitting this application, as an officer of this ministry, I certify that this ministry is hereby authorized and does request affiliation with Temple of the Goddess, headquartered in Los Angeles, California; and declare that we are subject to the authority thereof.
I further recognize that a debit to PayPal is given herein and will be made to Temple of the Goddess in the amount of $100.00 upon submission of this application. I also understand that application process does not guarantee automatic affiliation with Temple of the Goddess.
It is recognized that affiliation with Temple of the Goddess may be terminated at any time by an official act of either the Corporate Board of this organization or the IRS, and that such act will result in the immediate termination of affiliation.
